Crisis Symptom Control Guidelines
These guidelines are intended for â€Å"quick reference” lend oneself in crisis situations. They are designed to complement the more vast sections on the use of opioid analgesia and use of the spray driver.
Is the patient unavailing to swallow?
Is the patient vomiting?
Is the patient semi/ unconscious(p)?
If yes, use subcutaneous injection.
Pain
Breakthrough of pre-existing, opioid responsive pain
The following are not suitable for breakthrough pain:
Fentanyl patches
morphia opposed release tablets
Boost button on a syringe driver
Use oral morphine liquid or diamorphine injection
superman for breakthrough pain = 1/6th total opioid battery-acid in the last 24 hours
To convert doses:
90mg Oral Morphine = 30mg SC Diamorphine (3:1)
25 microgram patch Fentanyl = 30mg SC Diamorphine/24h
venereal infection should be effective within 30 minutes
If ineffective, consider borrowing
If repeat dose ineffective, consider advice from specialist palliative care team.

Nausea & vomiting
Parenteral drugs likely to be essential

Urinary tract infection
Constipation
Superimposed dread (use benzodiazepine)
*Assess likeliest cause:
Drugs (opioids, chemo.)†1st line Haloperidol
/ metabolic (urea, calcium)†second line Levomepromazine
Gastric stasis†1st line Metoclopramide
2nd line Cyclizine
Intestinal obstruction
No colic†Metoclopramide + dexamethasone
