PRESENTING PROBLEM:
Gracie Berntson is brought in by her perplex for a four-year thoroughly child care exam. Mother has no specific health concerns regarding common health, but does discuss some toileting issues that Gracie is having. Gracie corresponds her stools and as a result has stool staining on her underwear and large, chafed intestine movements. Mother states that Gracie is a healthy, developmentally normal four-year-old.
omen ANALYSIS OF PRESENTING PROBLEM:
Gracie has fears regarding having a bowel movement. She will hold her stool and then when she finally does have a bowel movement it is larger in diameter and painful to pass. This has been a bit of a vicious cycle. Gracie did not have this job when potty training; however, this has become more of an issue in the past year. Mother and father give Gracie prunes daily, juices, and water. This issue has been going away on for the past year.
No aggravating or alleviating factors that mother can pinpoint.
CURRENT MEDICATIONS:
1.Gummi Multivitamin 1 daily; culture dose taken 11/11/2012; taking for general health

Gracie is healthy and undergoing no current medical examination examination treatments.
